Schedule

H

Forms

Tablet 2mg, 4mg

Indications

Adult Dosing

Spasticity (MS, SCI, CVA)

Start 2 mg PO TDS; increase by 2 mg every 3–4 days to response. Usual: 12–24 mg/day in 3–4 divided doses.

Maximum 36 mg/day (higher doses: more side effects). Titrate slowly to minimise hypotension.

Acute muscle spasm (back/neck pain)

2–4 mg PO TDS for up to 2 weeks

Short-term only

Maximum daily dose: 36 mg/day

Pediatric Dosing

Age Range: Not established in children (< 18 years)
Dose: Off-label use in paediatric spasticity — specialist neurologist only

Limited paediatric safety data

Renal Dose Adjustment

CrCl / eGFRDose Adjustment
CrCl < 25 mL/minReduce dose — slower titration (start 2 mg OD or BD, titrate by 2 mg increments over longer intervals)
Calculate eGFR / CrCl →

Hepatic Adjustment

AVOID in significant hepatic impairment — hepatic metabolism (CYP1A2); risk of toxicity (hepatotoxicity and accumulation)

Pregnancy & Lactation

Pregnancy: Category C

Limited human data — animal studies show fetal harm. Avoid in pregnancy. Spasticity management in pregnancy: physiotherapy preferred; lowest effective dose of baclofen or tizanidine if necessary.

Lactation: Unknown

No data on breast milk excretion — manufacturer advises avoiding. Assume transfer based on pharmacokinetics; avoid.
