Scabies (5% cream)
Apply 5% cream to entire body from neck downward (include all skin folds, between fingers, under nails, genitalia, buttock cleft); leave for 8–14 hours overnight; wash off. Repeat application after 7 days.
Treat all household contacts simultaneously. Wash all clothing/bedding/towels at 60°C on day of treatment. Itch may persist 2–4 weeks after successful treatment (dead mite hypersensitivity) — does not mean treatment failure.
Head lice (1% cream rinse)
Apply to clean, damp hair, leave 10 minutes, rinse. Repeat after 7–10 days.
For infants: include face, scalp, and neck in 5% cream application. Adults/older children: neck downward only.

Generally considered safe in pregnancy — topical, minimal systemic absorption. Permethrin 5% is the preferred treatment for scabies in pregnancy. WHO and SIGN guidelines support use in pregnancy.

Topical use — negligible systemic absorption. Generally considered compatible with breastfeeding. Avoid applying to breast/nipple area.
