Indications

Adult Dosing

Treatment of influenza

75 mg PO BD for 5 days

Start within 48 hours of symptom onset — greatest benefit if started within 12–24 hours. Reduces duration by ~1 day and severity.

Post-exposure prophylaxis

75 mg PO once daily for 10 days (household contact)

Start within 48 hours of exposure

Seasonal prophylaxis (immunocompromised)

75 mg PO once daily for 6–12 weeks during influenza season

Specialist directed

Maximum daily dose: 150 mg/day (treatment)

Pediatric Dosing

Age Range: ≥1 year (treatment); < 1 year — specialist only
Dose: Weight-based for treatment (5 days BD): ≤15 kg: 30 mg BD; 15–23 kg: 45 mg BD; 23–40 kg: 60 mg BD; > 40 kg: 75 mg BD. Prophylaxis: half the treatment dose once daily.

Oral suspension preferred for children. Full-term neonates (specialist use — pandemic or severe illness): 3 mg/kg/dose BD.

Pregnancy & Lactation

Pregnancy: Category C

Influenza is more severe in pregnancy — benefit of treatment outweighs theoretical risk. WHO and national guidelines recommend treatment of pregnant women with influenza. Start within 48 hours of symptoms.

Lactation: Safe

Excreted in small amounts in breast milk; generally considered acceptable. WHO recommends treatment of breastfeeding mothers with influenza — continue breastfeeding.
