Indications

Adult Dosing

Hypertension

5 mg PO once daily

Usual maintenance. May increase to 10 mg after 2 weeks if needed. In CKD/elderly: start 2.5 mg.

Heart failure (stable)

Start 1.25 mg once daily; titrate by doubling every 2 weeks to target 10 mg once daily

Ongoing — mortality benefit in HF (SENIORS trial)

Maximum daily dose: 40 mg/day (though 5–10 mg most common for hypertension)

Pregnancy & Lactation

Pregnancy: Category C

Limited data. Beta-blockers in general can cause intrauterine growth restriction, bradycardia, hypoglycaemia in neonate. Use only if benefit outweighs risk. Labetalol preferred for hypertension in pregnancy.

Lactation: Caution

Limited data for nebivolol specifically — use with caution; monitor infant for bradycardia and hypoglycaemia
