- MRSA infections (skin, pneumonia)
- VRE infections
- Drug-resistant TB regimens
- Diabetic foot infections (resistant gram-positive)
Adult Dosing
Most infections
600 mg PO/IV every 12h × 10-28 days
Limit duration to 28 days due to bone marrow toxicity
Pediatric Dosing
Age Range: All ages
Dose: <12y: 10 mg/kg every 8h; ≥12y: 600 mg every 12h
Max/day: 1200 mg/day
