Indications

Adult Dosing

T2DM

5 mg PO once daily with or without food

Ongoing

Combination with metformin (Jentadueto)

2.5 mg linagliptin + metformin BD with meals

Maximum daily dose: 5 mg/day

Pediatric Dosing

Age Range: Not established in children
Dose: Not approved for paediatric use

T2DM in adolescents: specialist use only; metformin preferred first-line

Renal Dose Adjustment

CrCl / eGFRDose Adjustment
Any CrCl including ESRD and dialysis5 mg once daily — NO dose adjustment needed. KEY ADVANTAGE over other gliptins (renally excreted). Linagliptin eliminated primarily via bile/faeces.

Pregnancy & Lactation

Pregnancy: Category B

Limited human data; use only if clearly needed. Insulin preferred for gestational diabetes and T2DM in pregnancy.

Unknown breast milk excretion. Avoid or monitor infant.
