Tablet 0.5mg, 0.6mg, 1mg

Indications

Adult Dosing

Acute gout (low-dose protocol — preferred)

1 mg PO stat, then 0.5 mg after 1 hour

Total acute dose: 1.5 mg. As effective as high-dose with far fewer GI side effects (AGREE trial)

Gout prophylaxis (during allopurinol initiation)

0.5 mg PO OD or BD

3-6 months after urate target achieved

Pericarditis (adjunct)

0.5 mg BD (≥70 kg) or 0.5 mg OD (<70 kg)

3 months (acute); 6 months (recurrent)

FMF

0.5-2 mg PO daily

Lifelong

Maximum daily dose: 2 mg/day (therapeutic); 1.5 mg acute gout dose

Pediatric Dosing

Age Range: All ages (FMF)
Dose: FMF: < 5 years: 0.5 mg/day. 5-10 years: 1 mg/day. > 10 years: 1.5 mg/day
Max/day: 2 mg/day

Used in FMF throughout pregnancy with reasonable safety data. Avoid high doses. Folic acid supplementation advised.

Lactation: Caution

Excreted in breast milk; monitor infant. Generally considered acceptable for short-term use.
