Indications
- Heart failure with reduced ejection fraction (HFrEF) — mortality benefit
- Hypertension
- Post-MI left ventricular dysfunction
- Stable angina
Adult Dosing
Heart failure (HFrEF)
Start 3.125 mg PO BD; double every 2 weeks to target 25 mg BD (or 50 mg BD if >85 kg)
Must be clinically stable before starting; take with food
Hypertension
12.5-25 mg PO BD
Indefinite
Post-MI LV dysfunction
6.25-25 mg PO BD
Start 3-10 days post-MI when stable
