Indications

Adult Dosing

Heart failure (HFrEF)

Start 3.125 mg PO BD; double every 2 weeks to target 25 mg BD (or 50 mg BD if >85 kg)

Must be clinically stable before starting; take with food

Hypertension

12.5-25 mg PO BD

Indefinite

Post-MI LV dysfunction

6.25-25 mg PO BD

Start 3-10 days post-MI when stable

Maximum daily dose: 100 mg/day (HF, >85 kg); 50 mg/day otherwise

Pediatric Dosing

Age Range: Not established (specialist use only)
Dose: 0.05 mg/kg BD; max 0.35 mg/kg BD

Paediatric HF: specialist guidance only
